If you are ill and off work for longer than seven calendar days, you will need a doctor’s note.
For the first presentation of an illness you must discuss this with a GP. To do this, please phone 0131 549 7300 and ask for a call back from a GP. Please give the receptionist the details of your illness; a GP will then phone you back to discuss this with you. The GP may ask you to do a face to face visit or a Near Me (video) consultation before they are able to issue the certificate.
If you are requesting an extension to an existing sick line please ensure that you do not phone the surgery until your current sick line has ended. We are unable to forward-date sick lines. Please ensure you tell the receptionist that you are requesting an extension to your sick line.
For shorter periods of illness your employer will provide you with an SC2 self-certificate form.
