How to Register

We currently have an open registration list and would be delighted to welcome you as a patient, if you live within our practice boundary. If you are unsure if you live in Milton Surgery’s boundary, please contact the surgery to confirm prior to filling in the registrations forms. If you are outwith our boundary you can look for another surgery on the NHS Inform website. Please note, however, that NHS Inform lists the surgeries nearest you, but this does guarantee that your address falls within any specific surgery’s boundary. You can download the documents you need to complete registration – or call in and collect a ‘Registration Pack’ from reception at the surgery.

Please complete both separate forms and bring them to the surgery, along with the other proof of ID documents listed below:

Application to Register with a GP Practice – this is our patient registration form.

New Patient Confidential Health Questionnaire – this is an information sheet, giving details about your previous health.

Our staff are happy to help you with the registration process. To register you must provide identification for each family member.

Please understand that without the correct documents we will not be able to complete your request:

  1. NHS medical card (alternative to photo ID applicable to UK residents only)
  2. Birth certificate
  3. Photographic ID (passport, ID card or driving licence only)
  4. Proof of address  (ie mortgage/rental agreement, utility bill within last 3 months, bank statement, current employment documents)

You will be asked to complete a new patient confidential health questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your previous practice.

Your health records will be transferred to us from your current GP if you are registered with a GP elsewhere in the UK.

If you are newly resident in the UK we will not have access to your previous health records unless you provide these to the Practice.
